Registration and Payment Information
- Registration closes November 10, 2005.
- Registrations received after November 10, 2005 will be processed at the On-Site registration center.
- A check, money order or credit card information must accompany your registration. If paying with Bank Wire or Electronic Payment, the document from the bank MUST accompany your registration. Please include $20.00 service fee on all Wires.
- Registrations are not confirmed and classes are not reserved until payment is received.
- Government Employees: a complete Purchase Order may accompany your registration form. Incomplete PO's will not be processed.
- Payment must be in US funds only.
Cancellation Policy
- Cancellation requests must be in writing.
- A full refund will be issued if written cancellation is received prior to October 17.
- A 50% refund will be issued if written cancellation is received from October 18 thru November 1, 2005.*
- No refunds will be issued for cancellations received after November 2, 2005.*
- Substitutions are always welcome.
*You may apply the pre-paid amount toward a future CSI conference or seminar within one year. Those who do not attend and who have not submitted written cancellation will be responsible for the full fee.
